Admission and Registration

All parents must first set up a phone call or in person interview to ensure that Elements Preschool is a good fit.

If it is determined that it is a good fit, a tour will be scheduled. The parent(s) and the child must attend the tour. If both Elements Preschool and the family would like to enroll the next step is to fill out the paperwork and pay the deposit of half of the first month.

Once payment and paperwork is received your child is enrolled.

Prior to the first day, all paperwork must be submitted to include:

  • The Enrollment forms to include acknowledgment of parent handbook, sunscreen and field trip permission slips (found on forms tab)

  • The Elements survey (found on forms tab)

  • A signed contract (school year only.  I send this to you upon enrollment)

  • A wellness check signed by the doctor with the date and within the past 12 months (depending upon age.  If the child is younger than three years of age, we follow the Pediatrician’s schedule of 3, 6, 9, 12, 15 and 24 months well checks along with immunizations schedule (you have 30 days to bring this in)

  • A current immunization record within the past 12 months or, as example above (this must accompany your child on the first day)

If your child is on any medications, we will need the medication in the original container, labeled with your child’s name and birthdate, and a care plan (explains how to administer the medication) signed by both the physician and the parents.
